Nope. They have never said John's last name. I've read all the books minus the graphic novel, but they've never said it.

Yup, I've read the Graphic Novel (Pretty good by the way), and all the books, minus Halo: Evolutions, and they never say his last name. They just kidnap the kid when they're still really young, flash-clone a new one to replace the original I believe, and then I believe they just give them a new name, and their number.
